If you want to add or change the name of the father listed on your child’s birth certificate you need to contact the San Francisco Office of Vital Records, they are responsible for registering all San Francisco births.

You can add a father’s name to a birth certificate by filling out and submitting the necessary forms in person at the San Francisco office of Vital Records located at 101 Grove Street Room 105.

You will need to complete an Application to Amend a Birth Record – Acknowledgement of Paternity/Parentage, which you can find on the San Francisco Office of Vital Records website. This form must be signed by both parents. There is a $20 fee associated with this form.
