A Williams Complaint is a form parents must fill out when making a complaint about a textbook problem, teacher assignment, or facility issue to SFUSD. You must file the form at the school where the issue occurred. Williams Complaint forms only cover issues of textbooks, teacher assignment or facility conditions.

The form is called the Williams form because of a class action lawsuit filed in San Francisco. The plaintiffs alleged that San Francisco and state education agencies, including the California Department of Education (CDE) failed to provide public school students with equal access to instructional materials, safe and decent school facilities, and qualified teachers.
