What is the application I need to get the SF City ID card?
The application you are required to fill out in order to receive an SF City ID card depends on the age of the applicant. If the individual is 13 years old or younger, he or she must fill out this application. If the individual is 14 years old or older, he or she will need to fill out this application. Applications are also available in Spanish and Chinese. You will have to return the completed application to the City Clerk’s office in person in order to receive your City ID.
