How do I get a copy of a San Francisco birth certificate?

The San Francisco Office of Vital Records is responsible for registering all births in San Francisco. You can obtain copies of birth certificates from the San Francisco Office of Vital Records either by mail or in person at the Department of Vital Records. Their office is located at 101 Grove Street Room 105 – across from City Hall. They are open 8 am to 4pm except for holidays.

In addition to the individual listed on the birth certificate, parents or legal guardians may obtain copies of a birth certificate. Attorneys, adoption agencies, and law enforcement may also be able to obtain copies. For additional information on who can obtain copies visit the Department of Public Health Office of Vital Records website.

You can also submit requests for birth certificate copies to the San Francisco Office of Vital Records through the mail.
