The first step to requesting a transcript from any school within the San Francisco Unified School District is to call 415-695-5543 and follow the automated voice instructions.

SFUSD requires an original signature (on your transcript request form or request letter). You can either make a visit to the transcript office, located at 555 Portola Drive, room 380, or send a letter requesting a transcript. If you choose to send a letter, it must include the following:

Your full name and the name used in school
Your birthday
The name(s) of the school(s) from which you need a transcript.
The year you left school
$5.00 per each transcript you are requesting. Money orders, cash or check made out to SFUSD are acceptable.
