Can someone else pick up my certified copy of a document from the Office of Vital Records for me?
Yes. You must first complete the required application for copies, and have it notarized. Second send this notarized copy, along with a letter specifying who you are authorizing to pick up documents to the person you want to pick up your copies for you.
This designated person will have to bring the application, and payment, along with the letter authorizing them to pick up copies to the Department of Public Health for processing. The Office of Vital Records is located at:
