What is a medical home under Healthy San Francisco?
When you are enrolled in Healthy San Francisco, you will need to choose a Medical Home. In the Healthy San Francisco plan, a Medical Home is the facility, often a clinic, that will assign you your own physician for medical care.
The current Healthy San Francisco Medical Homes consist of San Francisco Department of Public Health clinics, San Francisco Community Clinic Consortium clinics and private providers.
